What are your recommendations for working up foley/indwelling cath urine cultures with multiple organisms?
The recommendation is that foley catherters should not be culture. However, urine collected from indwelling catheters are acceptable for culture. In those cases, two probable pathogens at levels of >10,000 CFU/ml may be significant and should be worked up. However, three or more organsims are considered contaminants. Also, the urine should be collected through the catheter and not from the collection bag. Guidelines for collection and interpretation of catheter specimens are available in Cumitech 2B (published by ASM).
