We currently use Vitek I and have had isolates of enterococcus that do not grow on the GPS cards nor on MHA or MH w/blood when performing manual testing. How would we resolve this issue in order to rule out VRE?
First I would confirm that your isolate is an enterococcus. To do this perform LAP, PYR and Esculin. You can do this using The BactiCard from REMEL or equivalent product manufacturerd by Hardy Diagnostics. Enterococcus will be Positive for all three tests and in addition will be catalase negative. Alternatively, you could perform ID on Vitek or API Strep Strip. If a confirmed enterococcus then it should grow on M-H agar. Could also try M-H with blood and incubate in CO2. If organism does not grow under any of these conditions then it probably is not an enterococcus
