Is testing for CSF measles IgM in CSF useful for diagnosis of SSPE
Diagnosis of SSPE is primarily made on clinical, epidemiologic and radiologic grounds with laboratory data serving largely a confirmatory role. Patients in general have elevated levels of anti-rubeola IgG and IgM antibodies in both serum and CSF, and I am not aware of any evidence that specific detection of differential titres of IgM antibody in CSF versus serum provides a more specific marker of SSPE than detection of elevated serum IgG in a patient with a clinical history compatible with SSPE and a history of acquiring measles infection during the first 2-3 years of life.
