Should susceptibilities be performed on Group B Beta Strept. in a Urine culture of significant amount?
We have worked out with our Infectious Diseases physicians that we will perform susceptibility testing for GBS from sterile sources on a routine basis. Such sources include blood cutlures, tissue, body cavity fluids, etc., but it does not include urines. As there is no known resistance to the beta-lactam antibiotics or vancomycin with GBS, our testing includes only clindamycin and erythromycin and D-test. One could argue that a fluorquinolone, such as levofloxacin or moxifloxacin, might also be tested routinely. As clindamycin and erythromycin are not recommended for treatment of urinary tract infections, this would leave only a fluoroquinolone to be tested from the urinary tract. I would suggest speaking to your ID service to see if a routine recommendation could be made and supported for this type of testing. But, in general, routine testing of GBS from urinary tract is not usually clinically necessary. We hope that we were able to answer your question.
